All on 4.

(See Fixed Teeth in One Day for a more patient friendly description of All on 4)

All on 4 is an immediately loaded full arch implant supported fixed restoration. It is suitable for cases of full arch edentulism or full arch failing teeth, either terminal periodontal cases or unrestorable teeth.

There are 4 implants, 2 anteriorly, generally in the lateral region and 2 angled distal abutments. The angulation allows the implant head to be located in the premolar or 1st molar region whilst avoiding anatomical landmarks such as the maxillary sinus and the mental foramen and Inferior Dental Nerve.

Patient immediately prior to all on 4 surgery

To summarise, a presurgical assessment and mock-up allows us to predetermine tooth location for aesthetics and occlusion.

On the day of surgery, under conscious sedation and local anaesthesia, any remaining teeth are removed, 4 implants are placed. Impressions and jaw registrations are recorded. The patient is discharged for a few hours.

In the lab the pre-op mock-up is 'married' to the impressions and processed to produce a 10 unit acrylic bridge supported on the 4 implants. This can then be fitted and the patient ends the day with a 10 unit fixed bridge.

Following osseointegration, (around 10 weeks) a definitive bridge, usually 12 teeth,  supported by a titanium bar is constructed.

Patient immediately after all on 4 surgeryTypical total cost £9950 allows for preoperative analysis and waxup implants, provisional and final bridge. Simple cases may be less expensive.

In edentulous cases, following a CT or conebeam scan, we can plan the treatment on computer. This allows optimal fixture placement and provision of a surgical guide, the bridge can be prefabricated and fitted immediately reducing time in surgery and giving minimal post-operative swelling or discomfort. Typically around £10,500 if the existing denture is satisfactory.
